Identify environmental risks
Make all possible modifications to reduce probability of falling
Notes:
When developing a fall prevention program, begin by identifying the people at risk for falls. This can be done using a fall risk assessment scale. Assessment should be done at least on a yearly basis and as needed, especially when there has been a change in health or mental status or a change in medications.
Identify any environmental risk factors using a home safety checklist.
Once problems have been identified, suggest or make possible modifications.
